Use the table below for reference when completing the fields on this screen.

Field/Button

Description

Employee code

The selected employee code displays in this field.

If the cost center feature is enabled in the Enterprise Installation screen, the employee list displays only employees associated with cost centers that the operator is authorized to access.

Description

Enter a description for the form. For example, if you are entering a form about parking space preferences, you might enter PARKING PREFERENCES.

Date required

Enter the date the form is required.

Comment

Enter any comment for the form.

Status

Select a status for the form: In progress or Completed

Date completed

Enter the date the form was completed.
