HR OSHA Cases Export File

Use the HR OSHA Cases Export File form to generate a .CSV file of injury/illness information that you can submit to the Occupational Safety and Health Administration (OSHA).

Beginning January 1, 2024, the Department of Labor requires establishments in certain industries with more than 100 employees to electronically submit injury and illness information to the Occupational Safety and Health Administration (OSHA).

Using this form, you can generate a .CSV file containing the same injury and illness information that you normally report using the OSHA 300 and OSHA 301 reports, and then submit the file to OSHA.

Once you specify the survey year, company ID, and establishment ID, select Export to generate the CSV file. The system assigns a file name consisting of the survey year, establishment ID, and "OSHACases" (for example, "2024_225006_OSHACases.csv").
