This form is used to set up and maintain mine information for MSHA (Mine Safety & Health Administration) reporting.

Mine information is stored by the MSHA mine identification number, allowing you to track summary information about the mine similar to MSHA. Information includes the mine name, operator, begin date, mine status (e.g. active, abandoned, etc.), the type of mine (e.g. surface, facility, etc.) and type of material being mined (crushed sandstone, rock, etc.), and the location of the mine.
